American Formliners was established, as a division of R&J Construction Supply Company, in 2008. Even though we have only been in business a short time, we have an extensive background, and have provided over 30 years of experience in the formliner business.

Our manufacturing staff has been developing and manufacturing technically advanced formliners and has a vast amount of valued experience in producing and offering innovative products. In addition to standard patterns we accept custom projects and have the capabilities to provide endless design possibilities with outstanding results.
John Kwiatkowski, Plant Manager – 37 years of experience in the manufacturing of architectural formliner molds and formliners. Many years ago John started his career as a Master Tool Maker. Then working as QC Inspector, Field Service Technician and other positions. Through the years he was able to gain a lot of professional experience in creating outstanding formliners, technical support and customer relationship.
Roger Kwiatkowski, General Superintendent – 33 years of experience in the manufacturing of architectural formliner molds and formliners. Positions Held: Machine Operator, Finisher A, Leadperson, Foreperson, QC Inspector, Supervisor.
Kim Cook, Administrative Assistant/Customer Service Representative – 20 years of experience with Customer Service in the formliner business.
Our plant, located at 645 Airport Road in Centralia, IL is over 20,000 sq.ft. of manufacturing facility, which houses tooling, thermoplastic processing and urethane processing.
Quality and Selection
Wide selection of formliners. We offer over a hundred standard patterns. If looking for a specific pattern, our craftsman can design and manufacture to your specifications. Our plastic liners are easy to install and strip.
Our liners can detailed for most pre-cast, tilt-up or cast-in-place concrete jobs. We offer five different material compositions, from single use high impact polystyrene plastic to our 100+ reuse premium polyurethane elastomer liners.
